Health Requirements for Starting School

Submit a Health Appraisal form completed by your physician. Must be completed by the first day of school. Minimum State of Michigan Immunization requirements:

4 – 6 years of age:

  • 4 DPT – 1 after 4 years of age           
  • 3 Polio – 1 after 4 years of age           
  • 2 MMR – 1st after 12 months of age                            
  • 2nd after 15 months of age           
  • 4 HIB – if entering before age 5 or entering the pre-school           
  • 3 Hepatitis B – vaccines in series           
  • 1 Chickenpox vaccine – after 1st birthday or date of history of the diseas

7 – 18 years of age           

  • 4 DPT – 1 in the last 10 years           
  • 3 Polio           
  • 2 MMR – 1st after 12 months of age           
  • 3 Hepatitis B (vaccine series)           
  • 1 Varicella (chickenpox) if given after 1st birthday and before 13 years of age. 2 doses if the vaccine is initiated after 13th birthday or date of history of the disease.
  • 1 Meningcoccal for children 11 years of age or older upon entry into 6th grade or higher.

If you have a religious or medical reason why your child cannot be immunized, a waiver must be signed before starting the first day of school.
