6th Grade Immunization Assessment

Beginning with the 2002-2003 school year, schools in Michigan will be required to assess and report the immunization status of all 6th grade students, in addition to the new school entrants, including kindergarteners, whom they currently assess.

This fall 6th graders must have documentation showing that they have had the following immunizations.

  • Two doses of MMR
  • Three doses of Hepatitis B, complete series of DtaP/Td with one dose in the last 10 years.
  • One dose of Varicella (or history of disease chicken pox)
  • One dose of Meningcoccal vaccine

The State of Michigan states that any student not completely immunized should be deterred from starting school. This requirement will help assure that all school-aged children are up-to-date and protected against all vaccine-preventable diseases.
